Angiotensin Receptor Blockers and Vascular Protection: Looking Back and Moving Forward
The last twenty years of clinical trial data have demonstrated the powerful potential of inhibitors of the renin-angiotensin system (RAS) to interrupt the cardiovascular continuum, and to provide powerful cardiovascular protection in patients at high risk, or those with established cardiovascular disease. This issue discusses the results of three recent clinical trials - VALIANT, Jikei Heart and UMPIRE- which demonstrate that angiotensin receptor blockers provide similar vascular protective effects to ACE inhibitors.
Cardiometabolic Risk Reduction: Charting the New Frontier
The articles contained in this issue outline the accumulating evidence that has implicated the endocannabinoid system (ECS) as a novel regulator of energy balance, satiety, abdominal obesity, and cardiometabolic risk, and the most effective ways to manage the cardiometabolic risk resulting from excess intra-abdominal adiposity.

Cholesterol Absorption Inhibitors Come of Age
This issue of Expert Opinions outlines current guidelines and clinical trial data on the “lower is better” theory with respect to LDL levels. It also provides compelling information on ezetimibe, a cholesterol absorption inhibitor that, when added to statin therapy, provides additional LDL-C lowering and LDL-C goal attainment, as well as significant reductions in hs-CRP.
Canadian Guidelines for the Management of Peripheral Arterial Disease (PAD)
This meeting report contains the key messages from the PAD Consensus Document, which was presented at the 2005 Canadian Cardiovascular Society Congress, and published in the Canadian Journal of Cardiology. It summarizes a number of issues with respect to PAD, including: epidemiology, prevalence, relationship and overlap with cardiovascular disease and cerebrovascular disease, risk factors and their management with both pharmacologic and non-pharmacologic strategies, as well as newer percutaneous and surgical techniques.

ASCOT-BLOOD Pressure lowering arm (BPLA) : results of a landmark clinical trial
The results of the blood pressure lowering arm of the Anglo-Scandinavian Cardiac Outcomes Trial (ASCOT) are the subject of this meeting report. This trial provides compelling evidence, as it shows that a treatment strategy using “newer” agents (amlodipine and perindopril) prevents more deaths than an “older” strategy (atenolol and thiazide diuretic).
New Evidence of Statin Therapy in Hypertension and Diabetes
This issue discusses the pleiotropic effects of statins, as well as the results of the ASCOT-LLA and CARDS trials, which demonstrated that statins provide powerful cardiovascular and cerebrovascular risk reduction in hypertensive and diabetic patients who are not overtly dyslipidemic.
C-reactive protein: A Biomarker for Targeting Statin Therapy
Inflammatory markers of atherosclerosis have become an area of intense interest, and high-sensitivity C-reactive protein (CRP) has been recognized as one of the most important. The Resurgence of the SOLVD Trials
This issue focuses on the unique ability of angiotensin-converting enzyme inhibitors to prevent cardiometabolic disease. The PEACE Study in Perspective: Acknowledging the ACE Inhibitor Ancestry
This meeting report focuses on the the Prevention of Events with Angiotensin Converting Enzyme Inhibition (PEACE) trial, the third in a trilogy of trials of ACE inhibition in patients with preserved LV function and cardiovascular disease. This discussion puts the results of PEACE into perspective in relation to the findings of other ACEI studies.
The VALUE Trial
This meeting report has the Valsartan Antihypertensive Long-term Use Evaluation (VALUE) trial as its subject, and provides a detailed analysis of the results. The review of the trial also focuses on how the trial did not fulfill its primary hypothesis, and what this means within the context of AT1 receptor blockade and cardiovascular benefits.
The REVERSAL Trial
The Reversal of Atherosclerosis with Aggressive Lipid Lowering (REVERSAL) trial is the focus of this meeting report, as it had been just recently published, and provided new insight into the potential differences between statins and their ability to alter the atherosclerotic process
Glycemic Targets and Pharmacotherapy: A Time for Change
This meeting report focuses on the Canadian Diabetes Association 2003 Clinical Practice Guidelines, which remain the only evidence-based national guidelines in the world. Dr. Subodh Verma interviews two key experts - Dr. Keith Dawson and Dr. Ehud Ur - who aided in the development of these guidelines. The interview touches on some of the salient topics within the Guidelines.
Canadian Working Group on Insulin Resistance: Third Annual Meeting Report
This meeting report reviews some of the key presentations and discussions from the third annual meeting of the Canadian Working Group on Insulin Resistance. The objectives of the meeting were to examine and discuss new scientific data on the mechanisms involved in insulin resistance, the organ-specific manifestations of insulin resistance, and new management concepts for type 2 diabetes.
